If you are applying for a green card (permanent residency) in the United States, you will need to complete Form I-693, the Report of Medical Examination and Vaccination Record. This form must be completed by a USCIS-designated civil surgeon.

**What is an Immigration Physical?**

An immigration physical is a medical examination required by U.S. Citizenship and Immigration Services (USCIS) for most green card applicants. The purpose is to ensure that applicants do not have any conditions that would make them inadmissible to the United States on health-related grounds.

**What the Exam Includes**

The immigration physical includes a complete medical history review, a physical examination, blood tests for certain conditions, a review of your vaccination records, and a tuberculosis (TB) test. Dr. Arevalo will complete the I-693 form based on the results of your examination.

**Vaccinations Required**

USCIS requires proof of certain vaccinations as part of the immigration process. If you are missing any required vaccinations, they can be administered during your visit. The specific vaccines required depend on your age and are determined by the CDC.

**What to Bring to Your Appointment**

Please bring a valid government-issued photo ID (passport preferred), your vaccination records (if available), any previous medical records related to the conditions being screened, and your immigration paperwork or case number.

**How Long Does It Take?**

The physical exam itself typically takes about 30 to 60 minutes. However, you may need to return for a follow-up visit to read TB test results or to receive additional vaccinations. The completed I-693 form is typically ready within a few business days.
